Mine first appointment was just prior to 6 weeks, but I'd had two losses previously, so they understood my concerns and got me in pretty early. At my doctor's office, the norrm is about 8 weeks.

Sharp pains are not a typical pregnancy symptom. The only way to know is to test after you have missed a period. You did not mention where you are in your cycle, but if you are late, POAS and see...
